Job Description:
A company is looking for a Primary Care Physician, Pod
Leader.
Key Responsibilities
Provide comprehensive, community-based care to vulnerable
seniors
Build long-term relationships with patients in a value-based care
model
Conduct home visits and manage a remote on-call schedule
Required Qualifications
Board Certified in Internal Medicine, Family Medicine, Emergency
Medicine, or Geriatrics
Eligible for enrollment to provide services to Medicare and
Medi-Cal participants
Full-time availability with regular hours of 8am-5pm, Monday
through Friday
Willingness to comply with COVID-19 vaccination policy
Experience in a value-based care environment is preferred
