Position Highlights:

• Position: Social Worker- Counselor, Community Health Center
• Location: 2500 Ridge Ave Evanston, IL
• Full Time/Part Time: Full-Time (40 hrs/week)
• Hours: Day Shift
• Required Travel: N/A What you will need:
  • License: LCSW required.
  • Education: Master's Degree from an accredited school of social work required.
  • Certification: BLS/CPR Certified.
  • Experience: Two years post-graduate experience preferred; healthcare, behavioral health, and/or mental health experience preferred
  • Skills: Experience with Electronic Medical Record (EMR) platform preferred. Experience with Microsoft Office Suite.
  • Other: n/a What you will do:
    • Provide case management services to adult patients in outpatient clinic
    • Provide counseling up to 50%
    • Coordination and referral to health and social welfare resources
    • Functions as integral member of a collaborative and interdisciplinary team
    • Will also serve as patient advocate
    • Crisis intervention and emotional support
    • Coordinate patient applications to medication assistance programs
    • Collaborate with patient support system as appropriate
